THIS AGREEMENT, effective upon acceptance by PCD, is entered into by and between Preferred Chiropractic Doctor, Inc., (PCD), with principal office located at 507 2nd Avenue South, Clanton, AL 35045, and the Provider whose signature and principal address appear on front page of this Agreement.

We obtain information we need directly from you on your enrollment form or from those you have authorized to provide information to us.

- : This includes identifying information such as name, address, phone number, and date of birth.
- Financial Information
- : If you choose to provide it to us for current billing purposes, we may obtain the type and the account number of your credit card along with expiration date and security code or the name of your bank and your checking account number. Credit card information is NOT kept on file and memberships will not auto renew.

We use information about you for business purposes such as maintaining your membership, and we may also use information about you to offer you other products and services.

We do not sell or rent information about you to others; however, we may share it with our contracted affiliated entities only as necessary to maintain your membership or our business operations.
